Documentation reviewed, failed to show that all staff received required quarterly life safety training. One staff member did not receive required training for the first and second quarter of 2024. Only two staff members out of nineteen received required training for the third quarter of 2024.
Documentation for the months of May, June and July 2024 failed to show that inmates are allowed one hour of supervised physical exercise or physical recreation at least three days per week.
