Notes and Summary

• The magistrate was being notified; however, the magistrate notification form was filled out incorrectly and did not advise the magistrate of the indicators documented on the mental health screening form. Also, the staff are not sending a copy of the screening form to the magistrate nor was the CCQ match reported as required by Article 16.22 of the CCP and minimum jail standards.
• The Screening Form for Suicide and Medical/Mental/Developmental Impairments was not being filled out, in its entirety, as required by minimum jail standards.
• The last health inspection occurred on January 22, 2018. This places the inspection past due by 9 months.
