The report notes, “During the review of a death in custody, it was determined that the magistrate was not notified as required by the directives noted on the Screening Form for Suicide and Medical/Mental/Developmental Impairments.
The jail administration confirmed, via e- mail, that the magistrate notification was not completed as required. The facility had this very same deficiency noted on the March 2, 2016 Annual Jail Inspection Report in that the staff were inconsistent in notifying the magistrate as required.”
