- The intercoms in holding cells 209, 210 and the detox cells did not work as designed.
- The last inspection of the fire panel was conducted on April 3, 2018 which makes it past due.
- The administration was unable to provide the VRSS reporting logs for 2018 or 2019 nor could it be proved that each prisoner was entered into the VRSS system.
- The administration was unable to produce the original classification instrument on several inmates for this inspector to review.
- The mental health screening forms were often missing from the files. This inspector could not verify if the local health officials were notified when necessary as required by minimum jail standards and Article 16.22 of the CCP.
- It was determined through documentation and video comparison, that one particular officer did not conduct physical face-to-face observations as it was documented on a shift observation sheet.
