Notes and Summary

• On the day of inspection, it was determined that a load transfer to the circuits was not conducted in January and June 2016.
• Staff is exceeding the 30 minute observations of inmates in detox and holding in excess of 30 minutes, at times.
• After reviewing an inmate medical file and interviewing the inmate, it was determined that an inmates medication was released to another inmate. The medications have not yet been refilled. Last received medication 7/16/2016.
• After interviewing inmates, it was determined that they have submitted requests for
  medical services and have not been seen by the doctor. After reviewing the inmates request to see the doctor, it was determined that one inmate had stated that he was having abdominal pain and was unable to eat. The request was sent on 9/7/2016 and an
  appointment has yet to be made. The second request submitted on 9/8/2016 was to receive treatment for HEP C and an appointment has not been made.
• On the day of inspection, it was determined that one jailer has resigned, leaving the
  facility with 4 jailers on staff. One jailer will be out on medical leave due to surgery. There is not a sufficient number of jailers to cover shifts.
• There is a leak in the main hall of the facility that needs to be addressed.
  The following issues were noted during the walkthrough of the facility:
  Dorm- The wall is rusting in the toilet area and the shower is inoperable. The sink in
  the closet has no hot water.
  A 3, there is no hot water in the lavatory.
  B1, there is no hot or cold water in the lavatory.
  B2, there is no hot water in the lavatory.
  B3, there is no hot or cold water in the lavatory.
  C dayroom, the intercom button has been removed and intercom is inoperable.
  C1, there is no hot water in lavatory.
  C2, there is no hot or cold water in lavatory.
  D dayroom, the parts for the shower have been removed and the shower is inoperable.
  D1, there is no hot water in the lavatory.
  D2, there is no hot or cold water in the lavatory.
• After reviewing a disciplinary report, it could not be verified when the inmate was offered the right to waive a hearing.
