Notes and Summary

• During the review of the face to face observations in the holding and detox area, this inspector observed that jailers exceeded the 30 minute limit between observations established by minimum jail standards by as few as 1 minute up to 45 minutes.
• During the review of medical records, it was discovered that the TB screening plan has not been approved by DSHS within the past year as required.
• During the review of the inmate mental health documentation, this inspector observed that the facility was not checking the CCQ for verification when the result was a “possible match”. This inspector confirmed several possible matches and found that the administration did not notify the magistrate according to CCP Art. 16.22 during the
  review as required.
• During the walk-through of the facility, this inspector found that doors for cells B1-5, B1-6, B1-7, B1-8, B2-5, and B2-6 did not open and close remotely as designed. Doors would try to close on their own after being slid open. The doors have to be kept locked open.
• During the review this inspector observed that the administration did not provide for at least 24 hours written notice
