This inspector reviewed multiple round sheets were the welfare checks exceeded the 30 minute requirement by as few as 1 minute up to 20 minutes.
• During review of face to face observations documentation for the Special Observation Sheets due to suicide concern, mental health concern or general health concerns, the staff exceeded the self-imposed 5, 15, 30 and other time check requirement(s). This inspector reviewed multiple round sheets were the welfare checks exceeded the selfimposed time limits by as few as 1 minute up to 11 minutes.
