- After reviewing a random sample of 30 minute observation logs of inmates in holding or detoxification cells, it was determined that jailers exceeded the 30 minute face to face observations by 2-17 minutes on a consistent basis.
- The jailers are not receiving the 3 hours of suicide prevention training every quarter as outlined in the facilities approved Mental Health plan
- The magistrate was not notified, in accordance with CCP 16.22, on an inmate that was displaying bizarre behavior.
- Jailers are not completing the supervisor notification, magistrate notification and mental health notification at the bottom of the Screening Form for Suicide and Medical/Mental/Developmental Impairments as required
- After reviewing a random sample of 60 minute observation logs of inmates, it was determined that the jailers exceeded the 60 minute face to face observations by 9-17 minutes on a consistent basis. Documentation reviewed also revealed that jail staff failed to conduct 30 minute observations in areas where inmates are known to be assaultive, potentially suicidal, mentally ill, or who have demonstrated bizarre behavior are confined.
