- Jailer Cristina Germany did not receive quarterly life safety training in the 2nd and 3rd quarters of 2017 as required.
- This inspector was unable to conduct a load test on the emergency power equipment on the day of inspection due to the jailer on duty not knowing the system and not having the training to operate the generator as required.
- Jailers are not notifying the magistrate when there are indicators on the screening form notating that there is credible information that the inmate may have a mental illness.
- Jailers are not completing the entire screening form as required. The bottom of the form is not completed and is being
left blank. There is no documentation to verify that medical was notified when an inmate reported having a TBI. After
interviewing staff, it was determined that a supervisor is not being notified as required. Staff were instructed to sign
the bottom of the form and document the date and time they completed the form. - Christina Chapa was hired on 9/17/2017 and does not have a temporary jailers license and has been working as a
jailer. The sheriff was not available and paperwork could not be submitted to TCOLE. Jailer Chapa was advised that
she cannot work as a jailer, interact with inmates and/or handle keys until she has a temporary jailers license. - There is no two-way voice communication capability between inmates and jailers in the holding and detox cells due to the fact that the intercoms were removed by a vendor and have not been replaced.
- The door to the recreation yard for inmates housed in multiple occupancy cells only works remotely and cannot be opened utilizing a key in the event of an emergency.
- After interviewing inmates and staff, it was determined that privileges are being taken away without due process. The
jail staff failed to follow the approved disciplinary plan on file with the Commission. After a telephonic conversation
with the sheriff, it was determined that the mail was being withheld due to the quantity of the mail received.
