- While conducting the walk-through of the Bi-State facility, it was determined that multiple single cells did not contain a seat, separate from the bunk, for inmates to sit as required by minimum jail standards.
- During the inspection, it was determined that licensed jailers from the Bowie County Sheriff’s Office working at the courthouse holding did not have the required quarterly life safety training.
- While reviewing inmate medical files, it was determined that medical staff failed to review prescriptions for two inmates taken into custody.
- While reviewing inmate medication administration records, several medical files (both day and night shifts) were identified as not having any documentation of the distribution of medications.
- While reviewing electronic observation logs, it was determined that jail staff are consistently exceeding the required 30 minute observations from 1 minute up to 27 minutes.
