- Cell doors within the facility were unable to be operated remotely as designed.
- During the inspector initiated fire drill, the smoke management system failed to automatically engage when the artificial smoke was introduced to the detection heads as required by Minimum Jail Standards.
- The Ansul hood system was last inspected on February 2019 which places the system four (4) months past due.
- During the review of the 30 minute face-to-face observations of inmates in holding and detox, it was determined that staff exceeded the 30 minutes required by minimum jail standards by as few as 10 minute and up to 30 minutes on a consistent basis.
- During the review of VRSS documentation, it was determined staff could not produce any documentation for uploads into the VRSS system since August 2019.
