Notes and Summary

After reviewing the screening form for suicide and medical/mental /developmental impairments and the Texas Uniform Health Status Update (TUHSU) form provided by Galveston County, it was determined that the inmate was taking medication for his
chronic infectious disease prior to arrival at Matagorda County but he did not receive medication nor did he receive medical care for his condition thereafter.
