• Could not provide documentation indicating that all jail employees received 2 hours of suicide prevention training as detailed in their Suicide Prevention/Mental Disabilities Operational Plan
• Unable to verify that full load generator tests were conducted
• Housing areas were observed as needing to be cleaned and have excess trash removed
  • Old side of the jail needs to be painted including rusty stairs in each dorm
  • Multiple light fixtures and vents were covered in paper
  • Multiple sink/toilet combs were dirty and rusty
  • These were areas of technical assistance during last annual inspection
