• On October 4, 2022, a person who was booked into Bailey County Jail did not receive a Suicide and Medical/Mental/Developmental Impairments Screening Form.
• The inspector discovered the “discrepancy” 14 days later despite the inmate receiving an “exact” match on her CCQ, requiring notification of the magistrate within 12 hours.
• It was further determined the person replied “yes” to four questions on the form, requiring notification of additional staff including Supervisor, Magistrate, Mental Health, and Medical.
