During review of restraint logs, it was determined that staff exceeded the 15 minute observation checks required by jail standards by 1 to 10 minutes on multiple occasions as well as up to 6 hours. Restraint logs were determined to be deficient in required details of types of observation checks that were performed. Multiple occasions of use of restraints lacked substantiation of incident reports. Technical assistance provided: Follow-up action required – Coryell County will conduct training with staff on the use of restraints and necessary documentation associated with utilizing the restraint log. Administration will provide documentation of use of restraint training and provide copies of restraint logs to complaint inspector Zachary Aguirre until further notice.
