- When reviewing the TCOLE license documentation, this inspector found two (2) jailers working the floor without a temporary jailer license.
- The jail did not maintain a 1:48 jailer to inmate ratio at all times due to the working of two (2), unlicensed jailers. Jailer Pennington was hired on 7/26/2016 and was not licensed until 9/15/16. Jailer Sims was hired on 8/24/2016 and was not licensed until 9/15/2016.
- During the review of the face to face observations in the areas where inmates are known to be assaultive, potentially suicidal, or display bizarre behavior, the jailers exceeded the 30 minute limit between checks as required by minimum jail standards
- During the review of the inmate mental health screening documentation this inspector
found that the facility was not informing the magistrate as required by the CCP Art. 16.22 and the approved mental disabilities/suicide prevention screening instrument. - During the walk-through of the facility this inspector observed in the west hallway what
appears to be evidence of a leak from the ceiling. The ceiling is beginning to buckle and a black substance appears to be growing on the tile.
