Notes and Summary
  • Medication Administration Records (MAR) reviewed failed to show that medication is distributed in accordance with written instructions from a physician.
  • During the review of one restraint log, it was determined that staff exceeded the required 15-minute observation checks by 4 to 12 minutes on all 19 checks conducted.
  • Administration was unable to provide the required documentation for the use of the emergency restraint chair to include; the events leading up to the need for restraints, the time the restraints were applied, the justification for their use, observations of the inmate’s behavior and condition, and the time the restraints were removed.
  • During the custodial death review of an inmate, it was determined that staff failed to conduct proper face-to-face observations of the inmate.
  • Documentation provided and reviewed for August, September and October 2024, failed to show that inmates are provided at least 1 hour of supervised physical exercise or physical recreation at least three days per week.
