A review of the generator logs revealed that a weekly load test was not conducted the week of August 4-10, 2024 and no load test was conducted for the months July and August 2024.
On 2 occasions the suicide questionnaire was not completed in its entirety as there was no indication of the magistrate being notified. On another occasion, the magistrate notification form indicates no notification necessary, while the questionnaire contained a litany of affirmative responses requiring notification.
On 1 occasion the magistrate was notified after 14 hours, exceeding the time frame required by CCP 16.22 by 2 hours. On 2 occasions the LMHA was not notified as required by affirmative responses on the questionnaire.
