During the review of medication administration records (MARS), this inspector on several occasions noted the ACC/DEC (Accepted/Declined) block on the MARS was marked “Out of Stock”. It was explained by administration that in some instances, staff marked the box “Out of Stock”, when the inmate had in fact refused the medication, therefore the MARS was not correctly documented.
During the review of MARS this inspector determined medication was not distributed to each inmate as ordered by the prescribing physician. It was determined there were 536 instances in which jailers indicated the medication was “Out of Stock”, this occurred out of 4639 instances in which medications were ordered to be administered. This inspector noted this to be a systemic issue.
