• During the investigation of a complaint, it was discovered that an inmate’s shower was turned off for disciplinary reasons. No documentation exists to indicate that the inmate was offered a shower every other day from May 2023 to December 2023.
• During the investigation of a complaint, it was discovered that an inmate was not offered recreation for at least one hour at least three days per week.
